Social Services Program Coordinator

Job Description

The employee is responsible for providing community bases social services to individuals, families, ad
unaccompanied youth who are homeless or at risk of being homeless. Reports to the Executive Director and provides case plans for interventions and supportive services.


Duties and Responsible
Develop and implement case plans including reviews and evaluations.
Provide information and refer clients for services as appropriate.
Participate in case conferencing meeting with other services providers.
Communicate effectively with parents/guardians, community partners and staff.
Demonstrate initiative in fulfilling performance responsibilities.
Prepare and maintain accurate records and documentation.
Assist with developing program outreach and public awareness materials.
Prepare monthly reports for billing for services provided.
Perform other duties as assigned.


Work Hours


Education and Experience
Bachelor’s Degree in Social Work/Human Services or a related field
Bilingual in English/Spanish is desired.
